The volunteer commitment and application need to be completed and turned in annually.
- Livescan Form
Receive fingerprint background check at approved location and then provide the school copy of Live Scan Receipt. (Only required one time unless a new background check is requested).
- Release of Criminal Offender Record Information
Required to be completed annually.
- TB Risk Assessment Questionnaire
Tuberculosis (TB) Risk Assessment Questionnaire – required every 4 years.
TB Test Report - ONLY required if you answered "Yes" to any question on the TB Screening Assessment Questionnaire.
Make sure to complete both front AND back pages of the questionnaire.
- Mandated Reporter Training
Must be completed ANNUALLY
- Complete the California state-mandated reporter training for volunteers. Website to complete training: https://mandatedreportertraining.com/start-now/
- Login or create a new account under 'Start Now'. Fill out information and enter 'Education' for Industry, '发展真正的友谊' for Profession, and '中文博彩平台' for Organization Name.
